New Client Form

Hi there! This new client intake form will help with the consultation process, understanding your hair/needs better, hair history, what products you are currently using, and planning for the future. Please be advised that I am not taking clients in need of a color correction and/or have box dye on their hair for lightening services. I am currently prioritizing clients who; have natural curls/waves, hair color for curls, creative coloring/vivid coloring, creative hair cuts/layered cutting/heavy face framing, interested in partnering with me & keeping their health of their hair a priority, respect their hair, respect my business, policies, and salon space.

All new clients will be required to fill this out before their appointment. If you’re interested in an appointment, please fill this out and I will be in touch with the next steps in the booking process or a referral. Thank you for your time & interest in my work!
